A tooth extraction with periodontitis is carried out when periodontal pathology has harmfully damaged a tooth, causing necrosis of pulp tissue.
In most cases, a dentist can extract the tooth. However, if the procedure becomes complicated, the intervention of an oral and maxillofacial surgeon will be necessary to carry out, and successfully, such extraction.

Factors That Trigger The Extraction Of Teeth With Periodontitis
Among the most frequent factors that lead to the extraction of teeth with periodontitis are the following:
- Chronic pyorrhea
At this stage, periodontitis has completely destroyed the gum and bone base, causing the affected tooth to move and it is much more feasible to extract it.
- Deep caries
In these cases, the piece cannot be devitalized or endodontic, because the condition has penetrated so much that it has damaged the roots of the tooth. In this way, the only possible treatment is extraction.
- Fracture of the tooth
When the trauma has been able to penetrate to such an extent to damage the root, or when there are those deep fissures that produce chronic infections, an extraction must be carried out.
- Integrated teeth that remain inside the bone or halfway out
These teeth or wisdom teeth are only extracted when they cause or are seen to produce repeated infections. Normally, they come out between sixteen and twenty-five years and, in most cases, there is not enough space for their eruption.
What Are The Phases For The Extraction Of Teeth With Periodontitis?
Before the extraction of teeth with periodontitis, the dentist will need to perform a series of steps. We’ll tell you!
- Medical history and medication
The possible general pathologies and the corresponding medication are analyzed, in order to determine if they have the possibility of harming the intervention. It is also observed if they interfere with the medication that is going to be given for the extraction itself, either antibiotics or anti-inflammatories.
- Radiography
The anatomy of the piece to be extracted, the restorations, such as fillings and reconstructions, and the state of the roots are studied. In addition, the existence of cysts or destruction of the bone surrounding the tooth with periodontitis is determined.
- Local anesthesia
A local anesthetic is applied to numb the area where the tooth will be removed, as well as a strong general anesthesia depending on the case. This last aspect occurs, especially, if several teeth have to be removed at the same time, making the manipulation of the tooth to be extracted painless.
General anesthesia prevents pain throughout the body and tends to cause dizziness or numbness during the procedure. Also, after it, the dentist or surgeon may place stitches and gauze over the wound to stop the bleeding.
- Surgical phase
In case of a normal extraction, the tooth will be manipulated so that it detaches from its base and can be removed.
Sometimes, the roots are curved or the crown of the tooth is very destroyed and must be separated by a file until proceeding with its extraction.
If the tooth is about to erupt or are embedded within the gingival area, as is the case with wisdom teeth, an incision should be made with the scalpel, separating the gum until the piece can be extracted.
Recomendaciones Post Extracción De Dientes Con Periodontitis
- Evitar fumar.
- Do not drink hot liquids during the first twenty-four hours, as the heat dilates the arteries and increases bleeding.
